(1) Vendor needs to provide community engagement services– Administrative Enrollment Site: a site that is not a Clinic Site but is part of the Clinic’s organization where the Clinic does eligibility determination and processes enrollments for health insurance (e.g., Medi–Cal, Covered state.– Ancillary Services: Limited solely to laboratory and basic radiology services (such as screening mammograms or chest x–rays)– Care Coordination: Service which facilitates a patient’s access to preventive, primary, specialty, behavioral health, or chronic illness treatment, as appropriate.– Dental Care Services: Medically necessary for the prevention, detection, and treatment of dental problems and includes dental support services, charting to dental records, administrative management and pharmaceutical.– Eligible Dispensary: An on–site dispensary with a valid State Board of Pharmacy permit to dispense pharmaceuticals, that meets all State regulations for clinic medication dispensing.– Enrollment System: Agency contracted web–based eligibility and enrollment system for the Program– Fee–for–Service: Method of payment for Included Services whereby clinics are paid one flat rate for each Participant visit, based on encounter Data submitted and the visit being part of included services.– Fee–for–Service: Method of payment for Included Services whereby clinics are paid one flat rate for each Participant visit, based on encounter data submitted and the visit being part of Included Services.– Health Plan: Refers to a Medi–Cal Managed Care Plan in County.– Included Services: Primary Health Care Services and Ancillary Services.– Medical Home: Clinic Site or Mobile Clinic selected by each Participant that a Participant intends to utilize as their regular source of Primary Health Care Services.– Medically Necessary: Services or supplies, which include Durable Medical Equipment as defined by agency in a PIN, are ones that meet the following criteria: a category of services required to be provided under this Agreement, and not specifically excluded, and recommended by the treating clinician.
