INSU-6189 - USA (West Palm Beach, Florida) - Long-Term and Short-Term Disability Insurance, and Term Life and Accidental Death and Dismemberment Insurance Services - Deadline August 18,2026

(1) Vendor needs to provide long–term and short–term disability insurance, and term life and accidental death and dismemberment insurance services to the government authority located in West Palm Beach, FL.– Long term disability insurance plan (LTD) will contain both employer paid core (provided to Medical HMO and Medical CHOICE participants only) and buy–up voluntary LTD benefits.– Provide an employer paid term life and accidental death and dismemberment (AD&D) plan and a voluntary, supplemental group term life and AD&D plan, as well as voluntary, dependent term life coverage, which is separated into spouse and domestic partner life, and dependent child life coverage.– Active Basic Life– Active Basic AD&D– Active Voluntary Supplementary Life– Active Voluntary– Supplementary AD&D– Active Voluntary Spouse/DP Life– Active Voluntary Spouse/DP AD&D– Active Voluntary Child(ren) Life– Retiree Voluntary Life(2) All questions must be submitted no later than July 20,2026.(3) The contract period will be for two years.
