(1) Vendor needs to provide asthma education services to the government authority located in Ohio.• Creation of digital educational content, asthma action plans, provider and family–facing tools, and delivery of a virtual educational webinar to improve asthma diagnosis, management, care coordination, patient compliance, and provider confidence.• Participate in Monthly Meetings with Primary Care Providers• Submit Monthly Progress Reports• Develop and Disseminate Provider Resources• Host Educational Webinars• Administer Educational Credits• Create an Asthma Toolkit for Centers• Provide Practice Coaching• Promotion of Asthma Educational Trainings(2) All questions must be submitted no later than July 30, 2026.
