(1) Vendor needs to provide mobile health expansion initiative services to the government authority located in Baltimore, MD. – Provide design, expand, or implement mobile, home–based, community–based, or place–based health access strategies for residents of county.– Mobile or Place–Based Clinical Services:• Preventive, primary care, oral health, behavioral health, maternal or postpartum, chronic disease, vaccination, screening, point–of–care testing, or low–acuity services delivered through mobile, home–based, school, community–based, or other accessible settings.– Behavioral Health and Substance Use Access:• Mobile, home–based, field–based, or community–based models that improve access to behavioral health assessment, crisis diversion, peer recovery support, substance use treatment linkage, follow–up, or ongoing care.– Care Coordination, Navigation, and Closed–Loop Referral Models:• digital health navigator, nurse care coordination, peer, or hybrid models that help residents’ complete referrals, attend appointments, address social needs, use telehealth tools, and remain engaged in care.– Digital Health, Remote Monitoring, and Micro–Access Models:• Technology–enabled strategies that support telehealth access, remote patient monitoring, digital health literacy, community–based telehealth access points, or chronic disease self–management.– Transportation–Enabled Access Strategies:• Transportation supports that are directly connected to care coordination, referral completion, and measurable access outcomes.(2) The contract period will be for four years.
