EDUCATION-17530 - USA (Baltimore, Maryland) - Diabetes Self-Management Education and Support (DSMES) Programming Service - Deadline July 16,2026

(1) Vendor needs to provide diabetes self–management education and support (DSMES) programming service to the government authority located in Baltimore, MD.   – This project will support the broader goal of developing a sustainable network of diabetes self–management support and education (DSMES) sites across the state with the capacity to offer services.    – Provider and staff engagement plan outlining strategies and materials for engaging and training providers and staff on DSMES integration and referral process.– Provider education, in–service trainings and promotional materials for providers about referrals.  – Develop a plan for provider engagement and training.– Train providers and staff using the developed operational manual.– Develop an outreach plan responsive to the priority population.– Awareness and outreach with the priority population.(2) All the questions must be submitted no later than July 10, 2026.(3) Pre–proposal meeting will be held on July 07, 2026.
