(1) Vendor needs to provide community paramedicine services to the government authority located in Richmond, VA.• Expand access to healthcare services for rural populations through community paramedicine and mobile integrated healthcare models.• Improve care coordination and continuity of care for individuals with chronic conditions, behavioral health needs, maternal health needs, or other complex healthcare needs.• Reduce avoidable emergency department utilization, hospital admissions, and hospital readmissions through proactive, community–based interventions, including clinically appropriate treat–in–place, alternative destination, and other innovative EMS care delivery approaches.• Strengthen collaboration among EMS agencies, healthcare providers, public health agencies, and community organizations to improve care coordination and referral pathways.• Improve access to preventive services, chronic disease management, post–discharge follow–up, medication management, and other evidence–informed interventions delivered in community settings.• Leverage telehealth, remote patient monitoring, digital health technologies, and other innovative approaches that improve access to care in rural communities.• Address barriers to healthcare access, including transportation, workforce shortages, geographic isolation, and social determinants of health.• Develop sustainable community paramedicine models that can be replicated or expanded to serve additional rural communities throughout state.(2) All question must be submitted no later than September 25, 2026.(3) A contract period will be for one year.
