(1) Vendor needs to provide statewide care coordination program services to the government authority located in Rockville, MD.– Provide consistent statewide care coordination services to ensure equitable access for all Children and Youth with Specific Health Care Needs (CYSHCN) and their families.– Provide standardized, comprehensive, culturally aware, and family–focused care coordinationService.– To include assessment of both the caregiver and the child with specific health care needs to determine level of need, assignment to the appropriate tiered level of support, and re–assessment at regular intervals.– Establish and maintain multiple modes of communications designed to meet the needs of referring agencies and clients/client representatives. – These modalities include in–person, phone (voice), phone (text), and video conferencing.(2) The contract period will be for one year.(3) Pre–bid meeting will be held on October 16, 2026.(4) All the questions must be submitted no later than November 6, 2026.
