(1) Vendor needs to provide diabetes self–management education and support enrollment enhancement initiative services to the government authority located in Baltimore, MD.• Identify workflow gaps, barriers, inefficiencies, and missed opportunities impacting enrollment.• Identify opportunities to improve access and participation.• Develop a workflow improvement plan outlining identified gaps, proposed solutions, responsible staff, and implementation timelines with the goal of increasing enrollments by 10%.• Develop or enhance standardized referral workflows and procedures to yield a 10% increase in enrollments.• Implement referral tools such as standardized referral forms, electronic health record (EHR) referral prompts, provider communication protocols, or cross–department referral processes.• Incorporate innovative approaches to support engagement.• Review performance data quarterly to identify trends and barriers.• Implement iterative workflow and enrollment strategy improvements based on findings.(2) All questions must be submitted no later than August 20, 2026.(3) A pre–bid meeting will be held on August 18, 2026.
