Adherence to this requirement is your responsibility and cannot be imputed to the Applicant. - All questions must be sent to bids@erateconsulting.com and must include the Applicant name and Form 470# in the subject line. - You must complete and submit Attachment A and/or B with your bid. Failure to submit this document will result in your bid being deemed incomplete for bid evaluation purposes. If you submit any additional bid documents where the proposed costs are inconsistent with the costs specifically proposed in Attachment A and/or B, then the cost specified in Attachment A and/or B will govern the Applicant's competitive bidding evaluation. For any Attachments insert your Name in column B1 and your SPIN in column C1. For "Facilities Owned/Leased", please indicate whether you are the owner of the transport. - SPAM and/or robotic responses not tailored to the Applicant's requirements will be deemed non-responsive and will be disqualified from consideration.
