(1) Vendor needs to provide case management and technical assistance services to the government authority located in Yonkers, NY. – Provide case management and technical assistance services to the residents of the agency and or managed eleven housing sites and the housing choice voucher program.– Provide case management assessment to include assessing residents’ overall health status, mental status, independent living skills, social supports, and service needs to include review of tenants that may be delinquent on the rental obligations.– Provide linkage to medical services and dental care.– Provide mental health and chemical dependency screenings.– Provide linkage to mental health and chemical dependency services.– Provide tenant home assessments.– Provide referrals to housekeeping training.(2) All the questions must be submitted no later than September 11, 2026.
