(1) Vendor needs to provide substance use disorder community–based outreach services to the government authority located in Washington, OR. – Provide SUD community–based outreach services to individuals discharging from a hospital emergency department who have problematic substance use or are diagnosed with a substance use disorder (SUD).– Outreach services are to reduce unnecessary emergency department admissions, coordinate post–discharge care, and improve access to community services and SUD treatment/recovery support in order to facilitate recovery and enhance overall well–being.– Community–based outreach services will be client–centered, trauma–informed, and strength–based.– Community–based outreach services will prioritize individuals who have problematic substance use, or who are recovering from substance misuse.(2) All the questions must be submitted no later than October 8, 2026
