(1) Vendor needs to provide doula care services program maternal and child health and behavioural health project to improve health outcomes, through equitable access to doula care services, to reduce socioeconomic disparities in birthing outcomes to the government authority located in St. Louis, MO.– Recruit and provide doula care for at least fifty (50) clients residing in the City– Refer clients to social services depending on their social barriers to health (food, transit, housing, child care, etc.). Ensure social service referrals are closed–loop– Closed–loop referrals are tracking systems that ensure a patient’s referral to a specialist or community organization is completed. The “loop“ closes when the receiving provider confirms the appointment occurred, records the outcome, and updates the referring provider, preventing patients from “falling through the cracks– Ensure completion of three therapy sessions with a mental health professional between intake and 1 year postpartum for mothers that screen positively on 5Ps.(2) All question must be submitted no later than September 22, 2026.
