(1) Vendor needs to provide medical sponsor service. – Provide services as a medical sponsor for the school–based health center.– Designate a staff person (manager) as primary point of contact to act as liaison between parties.– Medical sponsor shall provide primary medical to the children within the school district through designated medical sponsor staff.– Medical sponsor agrees to provide sick child exams and well child exams and associated treatment within the scope of practice of a primary care licensed medical provider through weekly clinics based.– Medical records will be the property of medical sponsor and are subject to medical sponsor and county’s policies of confidentiality regarding medical records and health information.(2) The contract period will be for one year. (3) All the questions must be submitted no later than October 1, 2026.
