INSU-6401 - USA (Tempe, Arizona) - Vision Services - Deadline September 24,2026

(1) Vendor needs to provide vision services to the government authority located in Tempe, AZ. – For a Vision Benefits Provider.– Coverage is provided for participating active employees and COBRA participants.– The plan year is July 1 through June 30. Eligible employees are those working a minimum of 20–39 hours part time and 40 hours per week full time.– Eligible dependents include an employee’s spouse, domestic partner, and dependent children to age 26.(2) All questions must be submitted no later than September 14, 2026.(3) The contract period will be one year.
