HEALTH-10600 - USA (Alameda, California) - Uninsured Health Program Service - Deadline August 24,2026

(1) Vendor needs to provide uninsured health program service to the government authority located in Alameda, CA.– Provide comprehensive Primary Health Care Services as described includes preventive care, routine and urgent primary care visits, chronic disease management (e.g., diabetes, hypertension), mild to moderate behavioural health services, nutrition education and services, ancillary lab services– Contractor shall provide all medically necessary basic laboratory andradiology services required to support Primary Health Care Services– The program formulary shall identify the pharmaceuticals for which manufacturer agency are available for Participants– Contractor shall encourage Participants to access primary care services through the Contractor for nonemergent needs and shall establish a mechanism to educate Participants on the appropriate use of emergency or urgent care services.(2) The contract period will be for one year.
