(1) Vendor needs to provide social, referral and case management program services to the government authority located in Chelsea, MA.– Community–based organization to manage referrals is to reduce health and safety risks by providing the city early awareness of identified problems and expanding staff capacity to respond with both codes enforcement and support services interventions.– Substance use disorders.– Emotional, mental, and physical distress or abuse.– Housing insecurity and rental assistance.– Food insecurity.– Need for Tax preparation.– Case tracking and follow–up.– Maintain confidential access to individual case situations, and work with the City to balance residents' rights to privacy.(2) All questions must be submitted no later than June 17,2026.(3) The contract period will be for one year.
