(1) Vendor needs to provide duty and physical examinations fitness services to the government authority located in Utah.• Provide pre–employment physical exams (PE) for Department and Law Enforcement exams (LE) for the jobs.• Approximately 150 DOT physicals, 150 law enforcement pre–employment physicals and 1200 TB tests.• Drug and Alcohol pre–employment and random testing for 750 DOT employees per year and 420 non– employees.• PSA (Prostate Specific Antigen – over 40 yrs)• TB test (PPD); pre–employment and annually• Baseline HIV• Baseline Hep B Antibodies• Baseline Hep C Antibodies• Chain of custody drug screen• Vision – Titmus test• Spirometry• Agency medical exam certificate (card)• Drug and Alcohol Testing (10–Panel):(2) All questions must be submitted no later than Sep 30, 2026.
