(1) Vendor needs to provide community guardian services to the government authority located in New York.– Provide ethical, effective, and person–centered guardianship services that reduce the risk of abuse, neglect, self–neglect, and exploitation.– Promote autonomy and self–sufficiency using the least restrictive alternatives possible.– Conduct multi–disciplinary assessments (i.e., medical, psychiatric, legal, and financial), incorporating collateral information and healthcare providers.– Guardianship case management and fiduciary advocacy.– Maintain adequate internal controls to safeguard client assets and prevent fraud, waste, abuse, and conflicts of interest.– Medical and behavioral health coordination.– Administrative compliance and data systems.– Offering robust fiduciary support, asset protection, and benefit management tailored to client needs.(2) All questions must be submitted no later than October 26,2026.(3) The contract period will be for three years.
