(1) Vendor needs to provide incident reporting management system.– The Incident Reporting Management System supports the standardized reporting, tracking and management of incidents occurring within operated facilities, bureaus and programs and will be used by staff to document, manage, investigate, and resolve incidents in a timely and consistent manner. – Provide an enterprise incident reporting and risk management platform that supports multifacility operations, standardized workflows, and advanced analytics. – The solution must use secure, scalable, highly available architecture that supports the following:– System can be cloud–hosted software as a Service (SaaS) (preferred), or– Hybrid (cloud application with approved state–managed integration/security components), or– On–premise (must meet all availability, scalability, and security requirements).– Solution must have data input and file validation functionality enabled and allow the use of customer–defined fields.– Solution must have the ability to convert all data into spreadsheets, graphs, PDF, and MS–Word utilizing a user–friendly interface.– Solution must have the ability to generate customized non–canned, ad hoc reporting on all data fields and allow users to analyse the incident and/or injury data that is recorded in the system for reporting purposes.– Solution must have a vulnerability management and remediation plan that includes ongoing monitoring; Risk assessment; Static Application Security Testing (SAST); and Dynamic Application Security Testing (DAST).– The system must provide a change–log of system incident record changes.– Solution must be able to interface with the PeopleSoft HR system, Human Resource software, to provide one source for information and reduce errors from freehand entry.(2) The contract period will be for one year.(3) All the questions must be submitted no later than October 30, 2026.
