(1) Vendor needs to provide chaplain services to the government authority located in Montgomery, AL. – To provide two church services each week at a time mutually agreed upon by the Contractor and the facility director or designee.– To provide funeral and graveside services as needed or requested.– To provide initial pastoral care consultation as referred by the treatment team and document in patient’s medical record.– To provide ongoing pastoral care consultations as referred by the treatment team and document in patient’s medical record.– service providers will participate in the evaluation of contracted services through methods selected by the facility.– These may include review of certificates, direct observations, audits, incident reports, chosen indicators or other suitable methods.(2) All questions must be submitted no later than August 18, 2026.
