Vendor needs to provide dentist services to the government authority located in Passaic, NJ.• Scope includes routine examinations, routine prophylaxis and care and dentures. The scope excludes oral surgery and procedures that require sedation.• Dental assessments of each resident within 90 days of admission.• Annual routine prophylaxis assessment or another time period that the dentist has established based on the needs of the resident and order of resident’s attending physician.• Dental services as medically needed. Procedures such as extractions or fillings, except in an emergency, require medical clearance by the attending physician.• Prepares, authenticates, and dates progress notes in legible detail of each visit at the time of the visit utilizing the facility’s software. Progress notes are maintained in the resident’s electronic medical record.• Must provide or arrange emergency dental care to relieve pain and infection.• Provide on–site services on a pre–approved schedule.• Attends medical staff meetings.
