Vendor needs to provide community health worker service to the government authority located in Hawaii. – Provide one Community Health Worker – to be responsible for each the following:• Outreach and Engagement. Achieve a goal of 50 referrals during the performance period and take part in a minimum of three (3) community outreach events (e.g., health fairs, in–services with organizations/partners, etc.).• Direct services. Aim for 50 in–person contacts/visits (either at home or within the community), with 60% of these resulting in direct health education or interventions throughout the performance period.• Care Coordination and Community Networking. Aim to initiate 50 referrals to partner organizations/agencies during the performance period.• Monthly performance management. Aim to submit one (1) monthly report and attend monthly meetings with the agency District Health Office liaison.• Final Report and Evaluation. Submit one (1) final report at the end of the performance period.• Supplies and Resources. Provide supplies and resources that support direct outreach and/or health education activities.– The agency shall identify perinatal women, children, and their families in need of health and social service supports by receiving referrals from other organizations and partners within the community, or through individual referrals and case findings.
