MEDI-8132 - USA (California) - Medical Transition Readiness Consultant Services - Deadline September 28,2026

(1) Vendor needs to provide medical transition readiness consultant services to the government authority located in California.– Consultant will focus structuring and facilitating agency process to identify strategies to minimize avoidable Medi–Cal coverage losses among agency clients, maintain behavioral–health treatment continuity, prepare for an increase in uninsured and restricted–scope beneficiaries, develop a Behavioral Health Plan benefit for the uninsured and restricted scope beneficiaries, assess and track the fiscal and provider–network impacts, and support implementation and outcome monitoring of key recommendations.– Conduct a project kickoff with agency executive leadership and designated partners.– Confirm project goals and objectives, scope boundaries, decision–making authority, escalation pathways, and success measures.– Finalize a detailed work plan, milestone schedule, risk and issue log, decision log.– Schedule and facilitate regular check–ins with the agency manager and ongoing workgroup meetings with identified key leaders; prepare agendas, materials, and decision and action notes.– Maintain alignment with evolving federal and state requirements.(2) The contract period will be for one year.
