STAFF-16913 - USA (Woodbury, New Jersey) - Audiologist Service - Deadline December 9,2026

(1) Vendor needs to provide audiologist service to the government authority located in Woodbury, NJ. – Provide audiological care services to the residents of home in accordance with local, state and federal regulations.– Audiologist will provide a copy of their license and current malpractice insurance.– The audiologist or his and her designee will notify the facility district nurse and nurse manager of any changes in services that affect the facilities residents including to any change in hearing care.– Services must include a complete in–house comprehensive audiological screening for each resident.– Scheduled visits monthly with residents to be seen at least yearly.– Emergency visits on a when needed basis.– The ability to use sigma care system for imputing documentation for the residents of agency.– Unlimited adjustments to hearing aids.– Must be responsible for the billing to medicare, medicaid and private pay.(2) The contract period will be for two years.
