(1) Vendor needs to provide transforming maternal health model analyze access service to the government authority located in MN. – Provide analyze access to whole–person perinatal care and care coordination for the transforming maternal health model in agency. – Conduct gap analyses on the current agency medicaid policy landscape to identify strengths, gaps and opportunities to improve integrated perinatal care and care coordination within the priority areas.– This could include literature reviews, community and provider engagement, and policy/cost analysis.• Priority area 1: Screening, referrals and follow up for perinatal depression, anxiety, substance use, and tobacco use • Priority area 2: Screening, referrals and follow up for upstream drivers of health• Priority area 3: Telehealth and home monitoring for gestational diabetes and hypertension, including related services, devices, and apps(2) All the questions must be submitted no later than November 6, 2026.
