(1) Vendor needs to provide independent assessment services to the government authority located in Albany, NY. • Provide a coordinated program of assessment services supporting oversight of the statewide health information network.• These services will include recertification, continuous monitoring, remediation validation, and authorized targeted assessments.• Assessing the design, implementation, and, where required, operating effectiveness of applicable controls andpractices.• Determine whether sufficient evidence supports adherence to each applicable requirement.• Identify compliance gaps, deficiencies, observations, risks, and enhancement opportunities.• Validate corrective action plans and remediation evidence.• Maintain complete workpapers and end–to–end traceability from requirement to conclusion.• Produce clear reports, dashboards, matrices, and executive presentations that track assessment performance and trends, support oversight and decision–making• Common Program Requirements• State Designated Entity Assessment• Assessment Planning and Project Management• Document Request List and Evidence Collection• Interviews and Control Walkthroughs• Control Assessment and Testing• Assessment Findings and Recommendations• Preliminary Findings and Factual Validation(2) All the questions must be submitted no later than November 12, 2026
