(1) Vendor needs to provide cloud–based reentry coordination and case management platform. • Provide a secure, cloud–based reentry coordination and case management platform designed to support incarcerated individuals and returning citizens as they transition from correctional facilities back into the community.• Solution shall serve as a comprehensive digital platform that enables correctional agencies, parole and probation personnel, workforce development programs, healthcare providers, housing organizations, educational institutions, and community–based organizations to securely coordinate services, track participant progress, and improve reentry outcomes• The solution must support end–to–end reentry case management, including intake and assessment, individualized care planning, service coordination, housing and employment referrals, healthcare tracking, reporting, analytics, and cross–agency collaboration• Conduct requirements gathering and business process analysis.• Configure workflows, forms, assessments, notifications, and reporting dashboards.• Establish user roles, permissions, and security settings.• Import and migrate existing participant and case–management data as required.• Perform testing, quality assurance, and user acceptance testing.• Provide go–live support and implementation assistance.• Deliver a production–ready cloud–hosted environment.• Provide implementation project management services.• Provide real–time visibility into participant outcomes and program effectiveness.
