(1) Vendor needs to provide community navigator positions services to the government authority located in Chelsea, MA.– Community Navigators Program is to provide and secure relationship–based linkages to a continuum of care for individuals with a substance use disorder and to guide individuals to other services and resources as needed.– Participants' engagement and enrollment– Family support network– General services– Recovery support– Harm reduction– Treatment referral– Transporting individuals to treatment and other services(2) All questions must be submitted no later than June 17,2026.(3) The contract period will be for three years.
