(1) Vendor needs to provide on–site audiology services on an as–needed basis to the government authority located in Sacramento, CA. – Provide a minimum of 24 hours written notice prior to bringing into the institution any hearing test equipment that has not specifically been authorized.– Provide notification of the dismissal to the agency or designee of all other institutions where said provider renders or plans to render services.– Perform comprehensive diagnostic hearing evaluations as clinically indicated.– Services shall include: pure–tone audiometry; speech audiometry, immittance testing, and otoacoustic emissions (OAE) testing. • Testing/services:– Pure Tone Audiometry and Speech Tests– Perform comprehensive diagnostic hearing evaluations on referred patients for the purposes of evaluating and assessing hearing loss, recommending hearing aids when medically indicated, and referring patients an Ear, Nose, and Throat (ENT) physician when clinically indicated.– Provide a comprehensive hearing testing and assessment report during the initial hearing evaluation.(2) All the questions must be submitted no later than August 27, 2026.
