(1) Vendor needs to provide mobile outreach and engagement service.– Mobile outreach is a critical strategy to address these barriers by providing low–threshold, community–based engagement for individuals who may not otherwise access services—particularly those who are unsheltered, justice–involved, or disconnected from healthcare and social service systems– Community data and stakeholder input underscore the need for expanded outreach services that prioritize trust, relationship–building, and consistent presence in high–need communities.– Agency is seeking qualified providers to deliver trauma–informed, culturally responsive, and recovery–oriented mobile outreach services– Services must emphasize dignity, respect, and connection, with a strong integration of peer recovery support and the meaningful involvement of individuals with lived experience• Engage individuals not currently connected to services, including those who are unsheltered, justice–involved, or otherwise marginalized• Provide consistent, person–centered outreach to build trust and relationships in community settings• Facilitate timely connections to substance use treatment, mental health care, primary care, housing, and social support• Assist individuals in navigating systems of care, including Medicaid enrollment, identification acquisition, and access to essential resources• Collaborate with community partners, including healthcare providers, emergency services, law enforcement, and community–based organizations• Ensure access to services for populations disproportionately impacted by substance use and related harms– provide low–barrier engagement, build trusting relationships, and facilitate timely linkage to substance use treatment, healthcare, housing, and recovery supports• Conduct outreach in high–need areas identified in collaboration• Engage individuals experiencing homelessness, justice involvement, or disconnection from care;• Utilize trauma–informed, person–centered approaches to build and sustain engagement;• Provide overdose prevention education and naloxone distribution;• Maintain a consistent community presence;• Have a streamlined process for transportation barriers to support access to services;• Spend a minimum of 75% of program staff time in the community doing street outreach(2) The contract period will be for three years.(3) All the questions must be submitted no later than August 6, 2026
