(1) Vendor needs to provide community education and outreach initiative engagement service.• Agency is seeking community–based and faith–based organizations serving rural Maryland communities to participate in a community education and outreach initiative focused on advance care planning and advance directives.• Participate in Training and Planninga. Identify staff, volunteers, or community leaders to participate in the project (at least 4–5 per organization)b. Participate in an agency –led orientation and Train–the–Trainer session on advance directives and advance care planning.c. Learn how to use the free digital advance directive tool, agency.d. Participate in planning and technical assistance with agency.e. Work with agency to adapt outreach messages, scripts, and materials so they are appropriate for the communities the organization serves.• Conduct Community Outreach – Each funded organization will conduct at least seven (7) community engagement activities focused on advance care planning and advance directives, including helping community members learn about and use the free agency tool• Collect Data and Participate in EvaluationFunded organizations are expected to support project evaluation by:a. Tracking community engagement activities, including the number of people reached and key demographic information, where possible.b. Collecting brief participant surveys provided by agency.c. Providing agency with requested information about completed activities.d. Sharing feedback about what worked, challenges encountered, and lessons learned.(2) The contract period will be for one year.
