(1) Vendor needs to provide medical director services to the government authority located in Canastota, NY. – Provide the following services, subject to district securing a consent to treatment and authorization for disclosure of medical information from the individual or the individual's parent or legal guardian.– Students consultant shall provide approximately 175 student physicals during the school year (September through June).– Pupil personnel services.– Provide approximately 90 sports physicals during the summer (July and August).– Personnel and occupational medicine consultant shall provide approximately 40 physical exams during the school year.(2) The contract period will be for one year.
