(1) Vendor needs to provide death audit and mortality verification services to the government authority located in Pennsylvania. – The selected provider will conduct at least weekly death audits of the agency’s benefit–recipient file, retrieve death certificates upon request, and conduct missing participant searches.– Identify deceased recipients and report any mismatches among name, date of birth, and Social Security number information.– Obtain and provide death certificates for deceased recipients identified through the audits, as requested by the agency. The Board estimates that it may request approximately 500 death certificates annually.– Conduct missing participant searches, as requested, that meet “due diligence” requirements for benefit mailings and plan terminations.(2) All the questions must be submitted no later than August 21, 2026.
