(1) Vendor needs to provide occupational health services to the government authority located in Plano, TX.• Provide occupational health services including but not limited to:1. Conducting post–offer physical exams for general employees, Police Officers, Firefighters, and Public Safety Communications employees in accordance with mutually agreed upon “protocols” based on appropriate standards for the position.2. Conducting annual physical exams for police officers, and selected employees from other departments in accordance with mutually agreed upon “protocols” based on appropriate standards for the position.3. Conducting DOT–regulated drug and breath alcohol testing (BAT) for safety sensitive positions.4. Conducting non–regulated drug and breath alcohol testing (BAT) for non–safety sensitive positions and non–DOT safety sensitive positions.5. Administering inoculations and vaccinations for communicable diseases.6. Providing correct and timely invoicing on a consistent basis.• Physical exam including complete health history; occupational history; height, weight, blood pressure, pulse, vision evaluation (including visual acuity, depth perception); hearing; and examination of head, eyes, ears, nose, throat, neck, thyroid, chest, heart, lungs, abdomen, hernias, extremities, neurological, skin, back and joint motions.• Testing– Serve as a collection site for breath alcohol testing and specimen collection for regulated and non–regulated drug screens as listed below. At a minimum, all testing should be conducted for the following drugs:• Marijuana• Cocaine,(2) The contract period will be for one year.(3) All the questions must be submitted no later than September 24, 2026.
