(1) Vendor needs to provide medical respite transitional housing services to the government authority located in Salisbury, NC.– Programs provide care, treatment, and rehabilitative services, including case management and therapeutic transitional housing assistance by contracting with community providers.– Provide food, shelter, and therapeutic services in a supportive environment to meet their basic needs, and ultimately, to facilitate the improvement of their overall health and housing situation.– Housing accommodations which are accessible to the Veteran 24 hours per day, 7 days per week.– Accommodations include a bed and other furnishings, such as a dresser, storage locker or designated secured space, and personal linens (pillows, sheets, blankets, towels/washcloths, etc.).
