(1) Vendor needs to provide case management software. • The case management system will handle case management (including case notes both billable/non–billable and assessment fields), demographic data entry, manage varied service plans by program, and store documents for the programs listed below:– Comprehensive Community Service (CCS), Children's Long–term Support Waiver (CLTS), Birth to 3, Community Support Program, Crisis, APS, Chapter 51/ED, Alcohol and Other Drug Abuse/ Recovery Court and Intake • Demographic data – to include: Name, DOB, Gender, Address, Race, Ethnicity, Phone Number, Parent/Guardian Name (where applicable and includes fields for address and phone number), Medicaid Number, Social Security Number, Primary Language and Marital status.• Document management – including medical records, prescriptions and case notes/narratives, court documents, provider statistic information, crisis reports.• Interfaces and integrations – with automatic or batch downloads/uploads as applicable.• Data migration – preference for a system that can import data from the following existing sources:– TCM, Laserfiche and shared internal document stores. • Provider portal – for direct program and fiscal interactions.• Vendor entry and maintenance: – Vendor Name, address, phone number, email signatory, etc.– Contracted services.– Contracted rates.– Billing Level – Master, Bachelor, Paraprofessional, etc.– Ability to approve individual providers for each vendor based on credential verification process.– Historical data for services, contracted rates – export/download capacity.• Provide ongoing technical support and resources as necessary for the system to function according to configuration and specifications for all users.
