(1) Vendor needs to provide regulatory survey service to the government authority located in San Jose, CA. – Provide regulatory survey vendors for timely access compliance reports.– Provide assistance with conducting required regulatory surveys annually.– Provider Appointment Availability Survey (PAAS).– Provide satisfaction survey.– Provider after–hours survey.– Advanced access assessment.– Data accuracy verification.– Ability to perform surveys to gather the necessary information and feedback from providers or practitioners to ensure regulatory compliance and to identify areas of opportunity related to the results.(2) The contract period will be for three years. (3) All the questions must be submitted no later than August 10, 2026.
