(1) Vendor needs to provide employee assistance program services to the government authority located in Gilbert, AZ.– Employee Assistance Program (EAP) insurance to support the districts needs.– Provide a comprehensive EAP program. The minimum program services include:• Provide a designated Account Manager and Account Management Team that provides exceptional customer service support via phone, e–mail and website.• Provide issue–specific face to face counseling services to employees and eligible dependents through professional counselors.• Provide a minimum of 10 one–hour face–to–face visits per issue, per employee or eligible dependent each calendar year.• Provide 24–hour per day, 365–day per year toll–free telephone crisis counseling.• Provide referrals to qualified, behavioral health providers in the District’s medical plan network upon request of the employee or eligible dependent requiring continuing care following the initial 10 visits(2) All questions must be submitted no later than September 14, 2026.(3) The contract period will be for one year.
