The Applicant respectfully requests that any contact be specific to this request. Any other contact is discouraged. MIBS and BMIC have been requested on this 470 in the event that any service or equipment in relation to this 470 and RFP is considered to by USAC to be designated as such. The quantity given on the antennas, connectors, and related components is in relation to the number of requested runs; the District understands the actual quantity may vary. Lewis County School District (the Applicant) is seeking proposals for a project to supply and install structured cabling at an eligible site. Bidders are advised that this project will be contingent upon district approval and the successful obtaining of Category 2 E-Rate funding, and the contract agreement between the Applicant and the successful bidder must recognize that contingency. Bidders should also understand that this RFP is essentially for time and materials pricing and the district reserves the right to scale this project back substantially or to cancel it entirely, regardless of the status of E-Rate funding. Contractor will supply pricing for time and materials to install up to a total of 512 +/- Category 6A plenum cable runs, each of an average of 250 feet at Lewis County Middle School, 337 LION LANE VANCEBURG, KY 41179, and terminated in vendor-supplied patch panels in IDF/MDF. All cabling and hardware at all locations shall be Panduit product or equivalent. The Lewis County School District will not accept the use of subcontractors on this project. A voluntary site visit will be held at Lewis County Technology Office, 65 Central Elementary VANCEBURG, KY 41179 at 1:00pm EST on September 9, 2026. This will be the only opportunity to ask questions or to receive answers to questions related to this RFP. Respondents choosing not to attend in so doing acknowledge their understanding that they were offered and voluntarily declined the opportunity. All proposals must include the following information: 1. Per- run pricing for each item included in your bid. Any discounting applied to your proposal must be reflected at the line-item level. 2. Two (2) reference sites where your company has performed a similar work as that being bid, including business name, contact name and contact information. It is preferable that at least one reference should be for a school corporation or library system within 100 miles of the Applicant. 3. Your E-Rate SPIN Number. (You must have a current SPAC form on file with USAC.) 4. A ready-to-execute contract which includes the proposal requirements and the E-Rate contingencies outlined herein. The Applicant requires that the selected vendor be willing to utilize the E-Rate SPI form of invoicing (discounted invoices). Bids should e-mailed to the office of Quentin Allen at quentin.allen@lewis.kyschools.us by 10:00am EST on September 24, 2026. All proposals must reflect the LCP (Lowest Corresponding Price), GSA pricing, and any available governmental unit discounts including existing state purchasing agreements or contracts when applicable. Any proposal referencing an existing state purchasing agreement or contract must include that information in the proposal. Bid prices must be complete for the services proposed and shall include all associated costs, even if the amounts are estimates based upon current applicable taxes, surcharges, or fees. No proposal can be submitted that includes equipment or components for equipment from companies deemed to provide a national security risk as defined by FCC Order 19-121.
