HEALTH-10298 - USA (Waterbury, Vermont) - Rural Health Transformation Program Service - Deadline July 17,2026

(1) Vendor needs to provide rural health transformation program service to the government authority located in Waterbury, VT.                                                     – The grant is to build stronger rural health networks, improve technology and shared operations, strengthen the rural health workforce and ultimately ensure that Vermonters receive the right care at the right time for an affordable cost. – To provide funds to help health care providers implement regional care strategies that coordinate services across a geographic area to improve access, quality, efficiency, and sustainability. – Services must represent start–up or catalytic investments that establish sustainable models of care while simultaneously addressing access, affordability, and improved health outcomes. • Care Innovation and Service Redesign Start–Up or Expansion Costs • Technology and Interoperability • Shared Services and Operational Efficiency • Hospital Designation of Service Transformation (2) All questions must be submitted no later than June 26, 2026.(3) The contract period will be one year.
