CASE-1814 - USA (New Jersey) - Area Plan Contract Case Management Program Services - Deadline November 20,2026

(1) Vendor needs to provide area plan contract case management program services to the government authority located in NJ.– Provide access, coordination, and management of services for clients approved for care/case management.– Care/Case management services will assure that services are being provided in accordance with the client approved plan of care.– Provide client advocacy, information and referral, service coordination, and other ongoing screening and monitoring services of the clients’ statuses to prevent premature institutionalization.– The service taxonomy defines a service unit as one half–hour.– The extent of the level of care (minimal, moderate, or extensive) is to be determined based upon the comprehensive assessment.– Care plan development: develop an individualized plan of care and services under a care management system based on a comprehensive assessment of the client’s needs and eligibility for services.– Care plan implementation: obtain formal and informal services according to an individualized care plan by coordinating existing services, authorizing payment for services, or purchasing services.– Care plan monitoring: determine quality and effectiveness of services provided to a client according to an individualized care plan and through periodic client contact.– Reassessment: ensure periodic review of client’s needs and the care plan to determine whether there have been changes in the client’s situation which warrant revisions to the plan, to determine if the client’s situation has stabilized, and to determine if termination of care management services should be initiated.(2) All the questions must be submitted no later than November 2, 2026.(3) The contract period will be for one year. (4) Pre–bid meeting will be held on October 28, 2026.
