Please reference Form 470 #270001219 on all correspondence. This is a request for switches, and any accompanying software, licenses, cables, connectors, mounting hardware and any other related items. USAC has advised applicants to include all three types of service on each Category 2 Form 470. ----- YOU MUST REFER TO THE ASSOCIATED RFP Document in EPC for compete details. This is not a formal "RFP," but provides details which should allow service providers to prepare more directed bids. It is important that service providers read the attached document to fully understand this project and prepare a more directed bid. Service Providers are responsible for monitoring the USAC EPC site for any updates to this posting. ----- QUESTIONS & ADDENDA: Questions related to the technical aspects of this document may be submitted via email to erate2027@nafcs.org and must be received by the deadline for submission of questions as identified in the preliminary RFP schedule below. The subject line of the email submissions should clearly state: FCC Form 470 270001219. Answers to any written questions or any additional information, revisions, or clarifications to the RFP will be provided in the form of an addendum to be posted with the FCC Form 470 on the Universal Service (E-Rate) website. It is the sole responsibility of the Respondent to check for any addenda that may be issued. No Respondent shall rely upon, take any action, or make any decision based upon verbal communications with any NAFCS employee. Inquiries are not to be directed to any staff member of NAFCS (except as expressly provided otherwise herein), and any such action may disqualify Respondent from further consideration for a contract resulting from this RFP. ----- SUBMISSION OF PROPOSALS: All proposals must be delivered via hard copy, AND include an electronic version of the proposal (including any required forms) on a USB flash drive in a sealed envelope, before the deadline in the preliminary RFP schedule section to: Bill Hamby, New Albany Floyd County Consolidated NAFCS Corporation, 2801 Grant Line Rd, New Albany, IN 47150 Any proposal received by NAFCS after the deadline will not be considered even if postmarked before the deadline. Only Respondents authorized representative may withdraw its proposal and any such withdrawal must occur in writing and prior to the proposal due date. ----- Preliminary RFP Schedule Activity Date Public Posting of RFP - October 8 & October 15, 2026 Date Form 470 Certified - October 8, 2026 Pre-Proposal Conference - N/A Deadline to Submit Written Questions - October 22, 2026 Response to Written Questions/RFP Amendments - October 26, 2026 Submission of Proposals - November 23, 2026 by 2:00 PM (local time) Anticipated RFP Award Recommendation - December 14, 2026 board meeting ----- NAFCS intends to award a contract to a responsible Respondent whose proposal is determined by NAFCS to be the most advantageous to NAFCS, taking into consideration price and the other evaluation factors set forth in this RFP. Award may be made to more than one Respondent whose proposals are determined in writing to be advantageous to NAFCS, taking into consideration price and other evaluation factors set forth in the request for proposals. Notwithstanding the foregoing, NAFCS reserves the right to instead reject all proposals and decide not to award any contracts under this RFP.
