(1) Vendor needs to provide array human services to the government authority located in Cape May Court House, NJ.– The objectives of the program. The objectives must identify at least one outcome objective that will measure the program’s impact on the client/youth (i.e., 50% of the clients/youth served will not be charged with an additional offense)– Identify any factors that would prohibit the admission of the client/youth to the program.– Identify the discharge planning process (if relevant to the program) and include reasons that would result in a negative discharge.– Staffing:1) The job description and credentials of all staff paid with County funds must be attached.2) Weekly hours of staff paid with County funds must be identified.3) Supervisory structure of the program within the Agency must be identified.(2) The contract period will be for one year.
