(1) Vendor needs to provide school age parenting program health services to the government authority located in San Antonio, TX.• Healthcare services accessed by students will be billed to the student or applicable payer through Medicaid, private insurance, or private pay, as applicable.• The selected provider shall develop a comprehensive plan outlining the healthcare services available to eligible agency students and the process for accessing those services• The proposed approach shall address the unique healthcare needs of pregnant and parenting teenagers and establish a coordinated process for connecting students with appropriate care.• The plans shall address the following:a. Healthcare Services: Identify the healthcare services available to pregnant and parenting teenagers, including applicable service locations and referral procedures.b. Care Coordination: Establish a proposed process for a designated agency School Age Parenting Case Manager to coordinate healthcare services for eligible students.c. Scheduling and Transportation: Outline procedures for scheduling appointments and coordinating transportation and accompaniment, as applicable, for students accessing healthcare services.d. Expedited Access: Recommend procedures to facilitate timely access to appropriate healthcare services for agency students.e. Implementation Plan: Provide a step–by–step implementation plan that leverages agency existing agency support structure and identifies the proposed roles, responsibilities, processes, and resources necessary to implement the plan.(2) The contract period will be for one year.
