QUALITY-2817 - USA (Utah) - Laboratory Services - Deadline June 30,2026

(1) Vendor needs to provide laboratory services to the government authority located in Utah.• Receive, log–in, and track samples as they are delivered.• Provide agency within two working days of sample receipt a copy of the completed chain–of–custody record that accompanied the samples during shipment a completed condition of samples upon laboratory receipt form, and a register form identifying the analyses that are scheduled for the samples.• Maintain and document chain–of–custody for each sample as it progresses through the laboratory, including disposal. Chain–of–custody forms will accompany each sample shipment documenting custody from field personnel to laboratory personnel.(2) All question must be submitted no later than June 26, 2026.(3) A contract period will be for five years.
