(1) Vendor needs to provide medical sponsor services.– Designate a staff person as primary point of contact to act as liaison between parties.– Provide staff administration, supervision for medical sponsor staff and designate staff to act as clinic coordinator, immunization coordinator, privacy official, quality assurance coordinator, laboratory coordinator and medical director.– Medical sponsor shall provide primary medical to the children within the school through designated medical sponsor staff.– Medical sponsor agrees to provide sick child exams and well child exams and associated treatment.– Medical records will be the property of medical sponsor and are subject to medical sponsor and policies of confidentiality regarding medical records and health information.– Medical sponsor agree to share client charts for the purposes of continuity of care and compliance with required data reporting.– Medical Sponsor shall provide services that are culturally and linguistically appropriate to the population and during services hours that are responsive to the population.– Medical Sponsor will submit claims to the applicable health plans for services provided to clients.(2) All the questions must be submitted no later than October 1, 2026.
