CASE-1786 - USA (San Francisco, California) - Ending HIV Epidemic Intensive Case Management Services - Deadline September 9,2026

(1) Vendor needs to provide ending HIV epidemic intensive case management services to the government authority located in San Francisco, CA.– Serious mental illness and chronic substance use disorder with limited to no insight as to the severity or impact of these conditions on self, friends/family, and community and not engaged with a behavioral health provider.– At risk of eviction; or health or safety hazard in current housing situation causing risk for institutionalization; living in a place not meant for human habitation (street, car, park, etc.).– Extended history of unsheltered homelessness and housing instability without resources and independent ability to resolve housing situation.– Limited tolerance for engagement in medical and social services inside of four–walls environments; and/or denial of service from one or more clinic or agency; and demonstrated need for intensive care coordination across multiple agencies.– Lack of healthcare coverage, financial entitlements, ability to sustain gainful employment, and food security(2) A pre–proposal conference will be held on August 17, 2026.(3) All question must be submitted no later than August 24, 2026.
