(1) Vendor needs to provide safety eyewear program service to the government authority located in Ames, IA.• Provide safety eyewear through multiple local distributors.• City employees will present proper authorization and prescription at the time of purchase stating the dollar amount and options to be provided• Frame style must be from the pre–authorized list only. Submit a frame list consisting of a wide range of sizes and materials, including non–conductive frames.• Eyewear distributors must be located in the agency and have extended evening hours at least one evening per week.• The safety eyewear provider should ensure that each City employee for whom services are provided has a correct fit and is provided with the correct eyewear prescription.(2) The contract period will be for one year.(3) All the questions must be submitted no later than July 13, 2026
