(1) Vendor needs to provide impaired optometrists program services to the government authority located in Frankfort, KY.– Help promote early identification and assist in the intervention, treatment, and rehabilitation of any credential holder or applicant who may be impaired by reason of illness or of alcohol or drug abuse, or as a result of any physical or mental condition.– Screening and interviewing new candidates for the impaired practitioner program.– Communicating with participant’s therapists regarding progress and treatment.– Making recommendations regarding an ongoing rehabilitation program or changes to a program.– Maintaining a case file on each participant with properly executed release forms.– Maintaining timely and accurate communication with the board on significant events that occur related to impairment and the ability of a credential holder to practice safely.(2) All questions must be submitted no later than September 10,2026.(3) The contract period will be for two years.
