(1) Vendor needs to provide chronic disease assessment, planning and training service to the government authority located in CO.• Improve health through evidence–based strategies that prevent, postpone and treat chronic disease.• The successful Offeror shall provide goods/services for all elements of the project track for which they are bidding.1. Complete an implementation plan.2. Revise the implementation plan annually at a minimum.3. Implement the approved plan.– Participate in the Rural Chronic Disease Consortium.– Complete quarterly progress reports.(2) The contract period will be for one year.(3) All the questions must be submitted no later than June 24, 2026
